Failure investigation is a systematic approach to the collection and analysis of evidence and data relating to the failure. In many cases an investigation may include: collation of data relating to the history and operating conditions of the item, visual examination, NDT, fractography, chemical analysis, mechanical testing and metallurgical analysis (e.g. microscopy and SEM). The scope of testing is dependant on the material, the type of item and the available evidence. When all of the evidence and data has been analysed a root cause of failure may be identified, allowing the client to reduce the likelihood of failure recurring.
